If you need an application for an absentee/vote by mail ballot, you can click on the appropriate link for your county.  For LA County, you can apply on-line.  For San Bernardino County, you can print the application, fill it out and mail it in.

Remember that applications for the November 4th election must be RECEIVED by the Registrar-Recorder by October 28.
